	Athletic Participation Policy - Revised for 2009-10 season. Includes Student and Parent Pledge.
	Athletic Request for Permission - Single page Request for Permission to Participate.
	Pre-Participation Physical Evaluation - Two-page form to be completed by physician prior to tryouts - one required per school year.
